Republikas laukuma klīnika provides medical termination of pregnancy up to 7 weeks of pregnancy. The procedure is carried out under the supervision of a gynaecologist after confirming the duration and location of the pregnancy.

A medical abortion, also called a medical termination of pregnancy, is a method of ending an early pregnancy using prescribed medication rather than surgery.

At Republikas laukuma klīnika, medical abortion is available only up to 7 weeks of pregnancy, provided that the doctor confirms that this method is medically appropriate.

The treatment involves two medicines:

mifepristone, which blocks the action of progesterone and stops the pregnancy from developing;
misoprostol, which causes the uterus to contract and expel the pregnancy.

The medicines must be prescribed by a doctor and used strictly according to the instructions provided.

How is the duration of pregnancy determined?

The duration of pregnancy is calculated from the first day of the last menstrual period and confirmed by a gynaecologist. An ultrasound examination may be necessary to:

confirm the duration of pregnancy;
verify that the pregnancy is developing inside the uterus;
exclude a suspected ectopic pregnancy;
assess whether medical abortion is suitable.

If the pregnancy is already beyond 7 weeks, medical abortion is not performed at Republikas laukuma klīnika. The gynaecologist will explain what further medical care may be appropriate.

Medical abortion regulations in Latvia

Latvian law allows termination of pregnancy at a woman’s request up to the 12th week of pregnancy. However, Republikas laukuma klīnika provides only medical abortion up to 7 weeks.

A referral for termination of pregnancy must be issued by a gynaecologist or general practitioner. Under Latvian law, the procedure may be performed no earlier than 72 hours after the referral has been issued.

Patients travelling to Riga should take this mandatory waiting period into account when planning their visit.

What happens during the first appointment?

During the first appointment, the gynaecologist will:

confirm the pregnancy and determine its duration;
assess whether the pregnancy is located inside the uterus;
review your health, allergies and current medication;
explain how medical abortion is performed;
discuss possible side effects and complications;
prescribe or arrange the necessary laboratory tests;
issue the required referral;
explain the mandatory waiting period;
arrange the next stage of treatment.

Please inform the doctor if you have severe anaemia, a bleeding disorder, adrenal disease, a serious chronic condition or if you use anticoagulants, corticosteroids or other regular medication.

What happens during a medical abortion?

The first medicine, mifepristone, is taken according to the doctor’s instructions. Misoprostol is used after the interval specified by the gynaecologist.

After taking misoprostol, you can expect:

lower abdominal cramping;
vaginal bleeding, usually heavier than a normal period;
blood clots;
nausea or vomiting;
diarrhoea;
chills or a temporary rise in body temperature.

Cramping and bleeding are expected parts of the process. The doctor will explain which pain-relief medicines you may take and how to monitor the amount of bleeding.

Lighter bleeding or spotting may continue for up to several weeks.

When should I seek urgent medical help?

Seek urgent medical assistance if you experience:

very heavy or persistent bleeding;
fainting, severe weakness or dizziness;
intense abdominal pain that does not improve after taking the recommended pain relief;
severe pain concentrated on one side of the abdomen;
fever or flu-like symptoms that persist;
unpleasant-smelling vaginal discharge;
no bleeding after taking misoprostol;
persistent signs of pregnancy;
any other symptom that causes serious concern.

Possible complications include an incomplete abortion, ongoing pregnancy, infection or heavy bleeding. In some cases, additional medication or a surgical procedure may be necessary.

Follow-up after a medical abortion

Follow-up is necessary to confirm that the pregnancy has been completely terminated and that no complications have developed.

The gynaecologist will tell you when to return and whether confirmation requires:

a gynaecological examination;
an ultrasound examination;
a pregnancy test;
additional blood tests.

Follow the instructions given by your doctor even if you feel well and the bleeding has stopped.

Does medical abortion affect future fertility?

An uncomplicated medical abortion does not normally reduce the chance of becoming pregnant in the future.

Fertility can return soon after the procedure. During the follow-up consultation, the gynaecologist can help you choose an appropriate method of contraception if you do not wish to become pregnant again.
